Consent and Acknowledgment Statement

By signing below, I acknowledge and agree to the following terms and conditions for participating in the coaching program with Agile Career Consulting:

1. Program Participation: I understand that participation in the coaching program involves engaging in one-on-one sessions, completing assigned tasks, and actively participating in discussions and activities designed to enhance my skills and career development.

2. Use of Media: I consent to the recording of pictures and video snippets of the coaching sessions in which I may appear. I understand that these recordings may be used for promotional purposes, including but not limited to marketing materials, social media posts, and website content. I grant Agile Career Consulting the right to use, reproduce, and distribute these media without further consent or compensation.

3. Payment Terms:

   • Non-Refundable Payments: I acknowledge that all payments made for the coaching program are non-refundable.

   • Installment Payments: If opting for installment payments, I agree to the following payment schedule:

     • A deposit of 60% of the total program fee is required prior to the start of the program.

     • The remaining 40% is due at one-third (1/3) of the way through the coaching schedule.

4. Commitment: I commit to participating fully in the coaching program, including attending scheduled sessions, completing tasks, and providing feedback as required.

5. Confidentiality: I understand that all personal information shared during the coaching sessions will be kept confidential and used solely for the purposes of the coaching program.
